Fay Nutrition and Discover Functional Nutrition: You most likely booked your appointment from my Fay Nutrition profile. Fay Nutrition processes all of the insurance claims for my practice, Discover Functional Nutrition.
If you have not already checked your estimated price per session, go to https://www.faynutrition.com/dietitians/jeremy-thararoop. Click on the Get your Price button on the left. Enter your information and Fay will link with your insurance company to determine your estimated price per session. It is important to know your cost per session. If you are ever billed for more than your quoted price per session, then you can reach out to Fay to correct the issue. Fay always honors your estimated cost per session.
If you have an issue or question with billing, Fay has a customer texting support number: 630-635-5950. This is a texting line only. You can also submit a billing inquiry at https://www.faynutrition.com/billing
Your appointment time is the time you reserve with your dietitian. Please give 24 hours' notice of appointment cancellation. If you give less than 24 hours notice, the Fay system charges a $75 cancellation fee.
Rescheduling: For appointments that are rescheduled more than 24 hrs. before the appointment can be done at no charge. If you have a same-day cancellation and if I can reschedule you in an open slot on the same day, the Fay system does not charge a late cancellation fee. However, if you have a same-day cancellation and I have no availability for that day, you will need to reschedule for a future date, which will incur a late cancellation charge of $75 for a same-day appointment cancellation.
No-shows: If you do not call, Fay instant message, text, or email your appointment cancellation, Fay will charge a no-show fee of $75. All charges for late cancellations and no-show appointments from Fay are irreversible, as I don't have access to their system.
Follow-up appointments: Follow-ups are generally scheduled as recurring weekly appointments to hold your time slot. Based on your progress and the level of support needed, appointments can be changed to biweekly or monthly basis. If you need to cancel for travel or vacation, you can cancel it in the Fay portal if it is more than 24 hours before your appointment.
Personalized Meal Plan templates and education: These will be attached and sent through the chat feature. I do not send anything that has your protected health information via regular email.
Fay is a HIPAA-compliant telehealth platform. You can submit food journals, send me a message via chat, manage your appointments, and meet via the conference link on your computer, tablet, or phone.To help you stay on track, Fay sends automated messages reminding you to log your food on my behalf. If we decide to use another way of food tracking, like MyFitnessPal, instead of Fay's food journaling, we can continue with the goals that we made together.
Onboarding Forms: Once you book an appointment, you should be prompted to complete a health questionnaire. Please complete these before your appointment.
Food Assessment QuestionnaireThe Vioscreen Food Assessment Questionnaire is a validated nutrition intake tool used to assess diet quality. It is used in research studies and can give great insight into your eating habits and what you can work on to improve your diet and meet your goals.Please complete the Vioscreen Food Assessment Questionnaire before your appointment. It takes about 20-30 minutes, and it is easy and picture-based. Clients have stated that it is easy, fun, and gives great information about their current dietary habits. A credit card is needed to hold your appointment slot. If a credit card is not added within 24 hours of booking the appointment, your appointment will be canceled.
If your credit card expires or is declined, Fay will reach out to me to let me know. Fay will need the credit card updated before we can resume appointments.
The initial session is 90 minutes long. Typically, the first half involves asking additional questions based on your completed intake form and food questionnaire reports and devising which nutrition tools would be best to support you. The last half consists of education and creating your nutrition plan based on your goals.
If you need to contact me, please use the HIPAA-compliant Fay instant messaging feature. Email is not HIPAA compliant, and I do not send any of your personal health information via email.
a. Our time is limited during the sessions. We may not cover all the possible questions that may arise between sessions. It is your responsibility to review all educational materials and handouts provided by your dietitian before reaching out to me for clarification.
b. If you can’t find the answer, then please reach out via Fay Instant Messaging.
c. I am available to respond Monday through Thursday from 9:00 AM to 6:00 PM MST and Friday from 9:00 AM to 12:00 PM MST. I will respond within 24-48 business hours. If available, I may respond outside of these hours, but this does not alter the availability listed above.
d. Fay Instant Messaging is for quick clarification of simple questions. More complex questions can be answered during your next scheduled session.
e. If you feel you need additional support between sessions, please contact me. I have an add-on service, but it is not covered by insurance and will be an out-of-pocket expense.
- You receive free access to Living Plate Rx or That Clean Life meal planning software as an active client if we choose use them as a part of your nutrition plan.
- Active clients are defined as having an upcoming scheduled appointment, and your previous appointment was within the last 30 days.
- If you become an inactive client and wish to continue accessing the Living Plate Rx meal planning platform, you can continue by purchasing a membership. Please reach out via email, Fay chat, or call/text 303-276-0282 to start your membership. That Clean Life meal planning software is not available as a subscription.
